Custom Forms - Setup & Configuration
Custom Forms can be used for a wide variety of tasks. This feature allows Members to create customized "checklists" that Users can administer via The Animal Welfare App to track routine inspections or procedures. Duties like opening/closing procedures, area inspections, equipment inspections, ethograms and much more can all be managed using the Custom Forms feature.
STEP-BY-STEP: How to Setup and Schedule Custom Forms

There are three primary steps to setting up and administering Custom Forms:
- Build a form with action items and/or questions;
- Schedule a Custom Form;
- Administer (fill out) your Custom Form
STEP 1 - Build Your Form
The very first thing that needs to be completed when creating Custom Forms is to build your form by creating a unique form name and then adding action items or questions to that form. These items can be anything that you want included on your form; and can often be applied to multiple forms.
Follow the steps below to configure your Custom Form:

Hover over the Documents menu.

Click the "Custom Forms" menu selection.

Click the "Form Builder" button.

Click the "add custom form" plus icon to create a new form.

Enter a name for the form and click the green check mark to submit.
NOTE:
A form can be scheduled so it is available to fill out on specific days, or you can choose to create a form "on demand". Be sure to click the appropriate check box when creating your form.

Select the Custom Form you want to add questions to from the left side panel.

Click the plus icon under the right panel to add questions/action items to the form.

Type your question/action item in the "Question" form field.

Select a Response Type from the dropdown list. Depending on your choice, you may also have the option to select an "Expected Answer". This is used to trigger alerts if the submitted answer does not match the expected answer.

Click the submit button when finished.
NOTE:
If you want any of your questions to be available for future Custom Forms, you can check the "Add To Library" box. This will save your question upon submit to be listed as a Library Item.
STEP 2 - Schedule Your Form
Now that you have a completed a form, the next step is to schedule a day and time it should be administered.
NOTE:
The steps below only apply to forms that you want to schedule, and NOT "on demand" forms.
Follow the steps below to schedule your Custom Form:

Select a form from the left panel that has been configured as a "scheduled" form. All forms that have been setup as scheduled will have a small clock icon to the left of the form name.

Click the green clock icon under the right panel to assign the list to an area/exhibit and set a day/time for the form to be filled out.

Click the plus sign (+) under the left panel to assign list to area/exhibit(s).

Select the appropriate days/time for when the schedule should be filled out.

Click the green check mark to submit.
STEP 3 - Administer Form
SCHEDULED FORMS
Once your Custom Forms have been configured, the final step is for your team to administer (fill out) the form. Depending on how it was configured, a form can be scheduled for a specific area and day; or it can be produced on demand.
Follow the steps below to administer a scheduled Custom Form:

Double click on the scheduled form you want to fill out. All scheduled forms will appear in the left hand panel under the area/exhibit they were assigned to.

Fill out the list of questions for your selected form.

Click the green check mark when complete.
ON DEMAND FORMS
Follow the steps below to administer an on demand Custom Form:

Click on the "Templates" tab in the right hand "On Demand" panel. This will show all configured "on demand" forms your team has created.

Double Click the on demand form you want to administer.

Click Yes to confirm you want to generate an on demand form.

Click on the "New" tab to view all on demand forms that have been generated.

Double Click the on demand form you want to administer.

Fill out the form questions and submit.
