Treatment Request (TR) Submission Form
The Treatment Request (TR) Submission Form is where front line caregivers begin the process of reporting an issue and requesting a treatment protocol to be initiated. Each request immediately becomes part of a communication and approval process where curatorial teams and veterinarians have the opportunity to review, amend, and approve/reject each request. All requests are archived in the Treatment Request Log where they will be assigned "start" and "complete" dates to track progress.
When creating a Treatment Request, there are three "types" of TR's that can be submitted:
- Area/Exhibit: used to request treatment on an entire system or habitat;
- Individual: used to target treatment to a single specimen; and
- Collection: used when targeting treatment to an entire collection.
Depending on which type you choose, the data fields available in the form will differ slightly. Below are instructions on filling out each type.
STEP-BY-STEP: How to fill out a Treatment Request Form
Applies to:
- Husbandry Member permission group.
- Facility Management permission group.
TR Submission Form - Area/Exhibit
To fill out a Treatment Request Submission Form for an Area/Exhibit, follow these steps:

Click the RED bell icon to launch the TR Form.

Select the TR Type from the drop-down menu.

Select the Location from the drop-down menu.
Note: You will receive a message in this step if you select a Location that has not had any water test results entered within the past seven days. These results can be important when determining treatments and dosages.

Toggle the "LSS Operational" switch to indicate whether the life support system is functioning. If you toggle the switch to "no" you will be required to enter details explaining the reason(s) it is not in operation.

Select an Organism/Disorder from the drop-down list. This list is configured within the Health Management module.

Write a brief description of the issue and be sure to include which species is being housed in the habitat so those reviewing the TR can make a fully informed decision regarding the request.

Select a Suggested Protocol from the drop-down menu. This list is also configured within the Health Management module.

If applicable, select a Secondary Protocol from the drop-down menu. This is optional.

Click the Upload Images button if you would like to add images to the TR.

Click Submit when the form is complete.
TIP: Whenever you see an "information icon" next to a drop-down menu, you can click on it to reveal details of the menu item; such as a Protocol.
TR Submission Form - Individual
To fill out a Treatment Request Submission Form for an Individual specimen, follow these steps:

Click the RED bell icon to launch the TR Form.

Select the TR Type from the drop-down menu.
- Select the Location from the drop-down menu.
Note: You will receive a message in this step if you select a Location that has not had any water test results entered within the past seven days. These results can be important when determining treatments and dosages.

Toggle the "LSS Operational" switch to indicate if the life support system is functioning. If you toggle the switch to "no" you will be required to enter details explaining the reason(s) it is not in operation.

Select the Name/ID of the the specimen for which the TR is being submitted.

Select the (1) Sample Type, (2) Organism/Disorder and (3) Result from the drop-down list. If one of these items are selected, the other two must also have a selection. The choice items for these lists are configured in the Health Management module.

Click the Plus icon (+) if you need to add more rows for Sample Types, Organism/Disorder and Results.

Write a brief description of the issue.

TR Submission Form - Collection
To fill out a Treatment Request Submission Form for a Collection, follow these steps:

Select the TR Type from the drop-down menu.
- Select the Location from the drop-down menu.
Note: You will receive a message in this step if you select a Location that has not had any water test results entered within the past seven days. These results can be important when determining treatments and dosages.

Select the Species for which the TR is being submitted. You can also include an estimated weight.
- [Optional] You can include a microchip if available.
